Essay

The thesis covers the investigation into improving the results of the surgical treatment of patients with inborn clefts of the upper lip and palate through improving the early primary surgical treatment. The author has developed and introduced into practice a one – stage surgery in this category of patients at their early age. The surgery provides for the early restoration of such functions as sucking, swallowing, chewing, breathing, articulating, as well as for the acceleration of the upper jaw growth. The above treatment has made it possible to decrease the postsurgical defects from 13,3 % to 4,8 %, and the postsurgical deformities of the upper lip and nose from 44,4-93,3 % to 23,8-47,6 %. The insufficient closure of the soft palate under early one – stage surgeries was observed in 4,8 % of patients, under early multi-stage surgeries – in 22,2 %, and under late multi-stage surgeries – in 66,7 % of patients. The author has also developed and introduced into practice the modification of Delorm surgery in patients ill with Robin sequence which contributed to a considerable improvement of conditions for breathing, swallowing and nutrition in all children operated on before they are one year old (86,6 %). The author suggests a clinical and surgical NLADVU classification which fully reflects all anatomic forms of the inborn clefts of the upper lip and palate. The above makes it possible to plan and evaluate the results of the surgical and integrated treatment.
