Rospopa Y. Principles and results of surgical treatment of wounds with extensive deep mechanical soft tissues injuries of the extremities

Subject - extensive mechanical wounds of extremities with the skin defect; aim of the research - to improve the functional, aesthetic results and to short the treatment of patients with extensive deep mechanical wounds of extremities by improving the tactics of performing the early plastic and reconstructive operations during the surgical treatment of wounds; methods - clinical, laboratory, planimetric, cytological, instrumental, questionnaire, statistics; scientific novelty - the objective criteria of dimensions of the wounds of extremities were justified, depending on the location, the possibility of skin grafting performing, the differential indications for the choice of methods of early reconstructive operations were justified during surgical treatment of extensive deep mechanical wounds of extremities, the influence of the form and timing of reconstructive and plastic intervention was proved on quality of life of patients with extensive deep mechanical wounds of extremities; results - it were proved the clinical efficiency of improving techniques of primary and secondary surgical treatment of wounds using ultrasonic cavitation and laser Doppler flowmetry. Developed methods and algorithms of surgical treatment of wounds allowed to reduce the total number of surgical interventions by 27,3 ± 7,3%, to reduce the duration of hospital treatment of 2,7 ± 0,3 times, to reduce the incidence of poor aesthetic results of treatment of 2,1 ± 0,9 times and improve the quality of life by 18%; introduced in medical practice in the teaching process; industry - medicine.
