Essay

The dissertation covers the efficiency upgrading of chronic diffuse liver diseases early and differential diagnostics in children and adolescents with up-to-date investigation methods: distance US palpation, MDCT-angiography and MDCT-perfusiongraphy. Liver US investigation with distance palpation and MDCT with MDCT-perfusiongraphy was performed for appraisal of complex diagnostic capabilities in 68 children and adolescents and differential diagnostics in 23 patients of comparable age group with chronic diffuse liver diseases. For the first time the mean quantitative values of US - elastography with share wave and MDCT-perfusiongraphy application in children and adolescents with unchanged liver parenchyma and in children with chronic diffuse liver diseases at early stage of fibrosis development were determined. Initial criteria of chronic hepatitis to liver cirrhosis transition were stated. Distance US palpation and MDCT-perfusiongraphy informativeness indexes were defined. It was proved that complex application of US - elastography and MDCT-perfusiography improves diagnostic accuracy of chronic diffuse liver diseases up to 95,6 %, sensitivity - up to 96% and specificity - up to 95,3 %.
