Essay

Object of the study: insulin resistance syndrome in type 2 diabetes mellitus (T2DM), and metabolic syndrome in men. Aim of the study: to increase efficacy of treatment in males with T2DM on the base of investigation of factors influencing on insulin resistance (IR). Methods of the investigation and equipment: clinical, laboratory, biochemical, immunoenzyme assay, instrumental, statistical methods; electro cardyograrh Юкард-200, rheograph Р4-02, ultrasonic apparatus PHILIPS HD11E. Theoretical and practical results: the obtained results substantiated an advisability of usefulness of the complex of indices that include status of carbohydrate and lipid metabolism, HOMA index, and indices of hormonal disorders, aiming to state the factors which promote beginning of IR, leptin resistance, hypercortisolemia in T2DM patients. The possibility of practical clinical employment of pharmacological decrease of IR in complex treatment of T2DM patients which improve carbohydrate and lipid metabolism indices was stated and confirmed. Novelty: it was established that IR is an important component of hormonal disorders in males, with IR extended in parallel with leptin resistance leading to body mass gain. For the first time was shown that the highest effect in relation to IR decreasing and treatment efficacy was achieved when antihyperglycemic drugs and hardly modified lifestyle were applied simulteniously. Degree of introduction: the practice recommendations have been used in clinical practice of department of endocrinology of the Ivano-Frankovsk regional clinic hospital and the Central district hospital. The main results of study have been proposed for use in the educational process of the department of endocrinology of State institution “Ivano-Frankovsk National medical university ”.
