Korobova O. Efficiency of a new method of prophylaxis of diabetic retinopathy in patients with type 2 diabetes after cataract surgery

Object of research: diabetic retinopathy (DR). Purpose: improvement of the prophylaxis efficiency for the onset and progression of diabetic retinopathy in patients with type 2 diabetes after cataract surgery by developing a new pathogenetically proved way based on the study of the system proteolysis-antiproteolysis condition, cytokine level and their correction by means of the specific medications. Research's methods: ophthalmological, enzyme-linked immunosorbent, biochemical, statistical ones. It was found out that in patients with type 2 diabetes having no signs of DR and with any stage of DR the level of MMP-9 and TIMP-1 in the blood and in the aqueous humor increased compared with the levels of these parameters in patients without diabetes, increasing of MMP-9, TIMP-1, TNF-a and IL-1b took place with increasing of DR severity and reached its maximum at the proliferative stage.The new method of diabetic retinopathy prophylaxis after cataract surgery in patients with type 2 diabetes, including the use of the HMG-CoA-reductase inhibitor - simvastatin 40 mg once a day in the evening, allowed to prevent DR onset and progression of mild NPDR in 70,2 % of cases, to receive absence of progression or minimal progression of DR in 91,5 % of cases for 3 years. Results have been implemented in 8 medical institutions of Ukraine, and the main provisions of which are incorporated into the program of lectures and workshops in 4 university medical institutions in Ukraine.
