Essay

The work is devoted to the сomplex study of vessels functional state in children with JRA, and the impact of various factors. In patients with established development of endothelial dysfunction, increased local stiffness of the brachial artery and the development of pathological changes in cerebral blood flow in the form of vasospasm were established. The functional state of the endothelium was evaluated by the group of indicators, such as endothelium-dependent vasodilation of brachial artery reactive hyperemia, shear strain rate and vasodilation. We have found a negative effect of the clinical features of disease, inflammatory markers, basic therapy drugs, blood pressure and its circadian profile on the functional state of vessels. The features of MTHFR gene polymorphism in patients and its association with clinical features of JRA, GC metabolism and its negative impact on the vascular endothelium were shown. We have prooved the necessity of the functional state of blood vessels diagnostics, blood pressure determination using daily monitoring and determination of MTHFR gene polymorphism in children with JRA.
