Essay

Object of the study: extra-and intracranial blood vessels of the brain. Aim of the study: to describe the state of cerebral blood flow and cerebrovascular reactivity in patients with type 1 and 2diabetes mellitus (DM), to determine the impact of cardiovascular autonomic neuropathy on the development of brain regulation disorders. Methods of the investigation and equipment: clinical, laboratory, biochemical, instrumental, statistical, Elegra SIEMENS (USA), electrocardiograph application module with Poly-Spectrum-Rhythm. Theoretical and practical results: The new data, which was found as a result of the study of cerebrovascular reactivity (CVR), show the disruption of autoregulation mechanisms which may be an important link in the pathogenesis of cerebrovascular disease development in patients with type 1 and type 2 diabetes mellitus. The modern methods of ultrasound diagnostics of CVR disruption in patients with diabetes mellitus were improved, the most informative non-invasive tests to detect these disruptions were determined. Novelty: the main changes of extra-and intracranial blood flow were defined on the basis of comprehensive study of cerebral hemodynamics in patients with type 1 and 2 diabetes mellitus. The decrease of the cerebrovascular reactivity in patients with type 1 and 2 diabetes mellitus was found through transcranial dopplerography with the use of vasodilatory and vasoconstrictive function tests. The impact of cardiovascular autonomic neuropathy on the state of cerebral blood flow and cerebrovascular reactivity disruption was determined.
