Zhdanovych I. Estimation of efficiency and prognostication of success of prosthodontic treatment of generalized periodontitis.

Dissertation is devoted to an actual question of modern dentistry – optimization of chart of prosthodontic treatment of patients with generalized periodontitis by estimation of its efficiency, development of algorithm of prognostication of progress of prosthodontic treatment, on the basis of study of features of the immunological state of oral cavity and state of fabrics of parodontium. The complex clinical estimation of the state of dental apparatus was provided taking into account the immunological and morphological changes at patients with generalized periodontitis, depending on efficiency of prosthodontic treatment. The role of strengthening of local immunity in inefficiency of prosthodontic treatment was proven (growth of the concentration of immunoproteins in the mouth liquid, lizocym, correlation of sIgA:IgA and level of pro-inflammatory cytokines). Morphological substratum of inefficiency of prosthodontic treatment are the destructive changes in fabrics of parodontium (violations of microcirculation, destruction of periodontal tissues, abscesses) on a background of low potential of fabrics regeneration (low level of angiogenesis). The chart of patients treatment was developed, which includes the improved methods of prosthodontic treatment with the prognostication of its efficiency that will allow choosing the most optimal method of prosthetics.
