Morpho-metric parameters of shoulder radial nerve topography were established on experimental stage of this work on 76 human corpses. In particular, the radial nerve exit point from the same-name channel in adult men of meso- and brachymorphic constitution and in all women represents a stable topographic-anatomical landmark separated by a distance D-E and expressed by a coefficient of 1,3 E-F where D is the radial nerve exit point from groove; E and F - peaks of lateral and medial epicondyles of humerus correspondingly. Following the late results analysis of operative service of 197 patients with humerus fractures, the algorithm of differential approach to choice of osteo-synthesis optimal technology with consideration of patient's constitution specificity, fracture kind and type, and clinical-instrumental neurological examination results was developed. Assessment of late results of humerus fracture surgical service when shoulder radial nerve topographic-constitutional specificities are considered showed the efficiency of use of intramedullary blocked osteo-synthesis and plates with angular stability. At the same time, humerus fracture operative service with none consideration of shoulder radial nerve topographic-constitutional specificity points to relative risk of said nerve's neuropathy development 7,2 (95% CI 4,2 - 12,5) with chance correlation of 13,5, and 43% of the absolute risk.
