Radionova S. A catatonic schizophrenia (clinic-pathogenetic and immunomethabolic features, principles of treatment)

In the dissertation it is carried out complex of the common-clinical, psychopatho-logical, pathopsychological, immunological, biochemical and instrumental inspection of 126 patients with catatonic schizophrenia (CS). A basic group (64 patients) in the com-plex of medical treatment got additionally immunoactive preparation polioxidonium and metabolic active preparation reamberin, the group of comparison (62 patients) got the generally accepted facilities. Clinical and psychotic structure CS is characterized by a predominance of hypokinetic variant of the disease manifestations mutism, passive nega-tivism, wax flexibility and torpor. In hyperkinetic variant dominated CS palinergy symp-toms and verbigeration. Hallucinatory, delusional and affective syndromes are divided according to the type of clinical course of CS. It was set that catatonic schizophrenia is accompanied by changes in the immune, metabolic and microhaemodynamic perform-ance, characterized by the presence of secondary immunodeficiency, development of immunotoxic syndrome, development disorders of blood cytokine profile, decreased ac-tivity of interferon along with the fall of the efficiency of energy metabolism and inhibi-tion of antioxidant defense system and nitric oxide metabolism in background activation of lipid peroxidation and endogenous metabolic intoxication syndrome and microhaemo-dynamic disorders. It was set that the use in treatment of patients with catatonic schizo-phrenia immune and metabolically active therapy provides a parallel improvement in clinical and imunometabolic, microhaemodynamic indexes. Clinical efficiency of appli-cation of an offered medical complex is found out by more high quality formation of re-missions and reduction of the general positive effect of quantity of psychotropic prepara-tions necessary for achievement.
