The dissertation is devoted to the clarification of mechanisms of metabolic cardiomyopathy (MCMP) in type 2 diabetes mellitus (DM-2) in patients with overweight; the definition of the role of adipokines leptin and resistin, proinflammatory cytokines interleukin-1? (IL-1?) and interleukin-6 (IL-6) in the development of myocardial lesions. It is shown that the development of diastolic dysfunction (DD) of left ventricle myocardium in patients with MCMP against the background of DM-2 is associated with abdominal obesity. As a result of cluster analysis patients with MCMP against the background of DM-2 were divided into 2 groups depending on MCMP intensity that varied due to severity of clinical symptoms and DD degree. The threshold of division into MCMP intensity groups according to body mass index was 28.47 kg/m2. In both groups of patients a significant relationships between clinical and laboratory parameters and DD were revealed, however these relationships were more apparent and numerous in the group of patients with evident MCMP. It was found that an increase of body weight causes the significant elevation of leptin, resistin, IL-1? and IL-6 levels. The relationship between leptin, resistin, IL-1?, IL-6 and DD indicates the impact of increased levels of these cytokines and adipokines on the formation and progression of MCMP against the background of DM-2 in patients with overweight.
