The dissertation is devoted to the clinico-pathogenetic role of leptin, resistin, tumor necrosis factor-? (TNF-?) and insulin-like growth factor-1 (IGF-1) in the mechanisms of progression of non-alcoholic liver disease (NAFLD) in patients in combination with type 2 diabetes mellitus (DM), that increases the efficiency of diagnostics in these patients. It is defined the pathogenetic role of leptin, resistin and TNF-? in the development of syndromes of cytolysis, cholestasis, insulin resistance and dyslipidemia, as well as the role of IGF-1 in the reparative processes in the liver, disorders of glucose, lipid metabolism and protein synthesis in patients with NAFLD against the background of type 2 DM with normal body weight and obesity. It is shown that obesity increases the impact of adipokines and IGF-1 on the indices of investigated forms of metabolism in patients with NAFLD and type 2 DM. It is established that the identified of high levels of leptin, resistin, TNF-? and low levels of IGF-1 allows you to set the early risk of progression of functional disorders of the liver and lipid and carbohydrate metabolism disorders, that improves the diagnostics in this category of patients.
