Essay

Dissertation work is based on clinical material (102 patients with rectal cancer of stages II-III complicated with intestinal obstruction) and presents a new solution of scientific task of modern oncoproctology, namely, elevation of the number of sphincter-preserving operations and improvement of quality of life of patients with rectal cancer complicated with large bowel obstruction. A new method of endoscopic recanalization of rectal lumen upon complete tumor obstruction was developed, and the following algorithm for treatment of this group of patients was proposed: firstly, endoscopic recanalization of rectum has been performed and entero- or colonosorption sessions have been carried out; secondly, after achievement of intestinal decompression and elimination of main endogenous intoxication symptoms, chemo- and radiotherapy with enterosorption has been performed, and then a planned surgical sphincter-preserving treatment has been done. If there were present poor prognostic factors, after performance of surgical treatment the patients were treated with chemotherapy. It is demonstrated that the developed algorithm for treatment of patients with rectal cancer complicated with large bowel obstruction allows significantly elevate the number of sphincter-preserving operations without worsening the prognosis of the main disease.
