Essay

The thesis presents a new solution for the vital problem of improving the efficiency of diagnosis and prediction of the disease course in children with ReA. Endothelial dysfunctions have been established, which occur three times more frequently during the chronic course than during the acute one (p<0,001). It was revealed, that disorders of the functional state of the microcirculatory bloodstream are directly dependent on the type of the ReA course. During the acute course they account for 68,7% (p<0,05), during the recurrent one - 88,2% (p<0,05), and the chronic - 94,4% (p<0,001). It has been demonstrated that the direction of changes in reographic values for the acute and recurrent courses are compensatory, and for the chronic course decompensation in peripheral hemodynamics is observed. It has been shown that changes in joints depend on the nature of the disease course. Predictors of recurrent and chronic course of ReA in children have been established and the prediction algorithm for the ReA course nature has been developed, which allows us to select the group of children that has chronic disease course.
