Essay

Object - bronchial asthma in school-age children; purpose - to improve the diagnosis and treatment of asthma in childhood based on the study of gene deletion polymorphism of glutathione transferases M1 and T1 with the subsequent development of individual approaches to the tactics of the treatment; methods - clinical, laboratory, molecular genetic, sputum induction, immunological, biochemical, statistical; Results - found that deletion polymorphism of genes GSTM1 and GSTT1 is associated with more severe disease, and its presence is accompanied by some clinical signs, and laboratory and instrumental performance; for the first time it has been shown that deletion polymorphism of genes GSTT1 and GSTM1 is associated with preservation of subclinical local inflammation of the bronchi during a period of clinical wellbeing, that dictates the necessity of carrying out a correction of standard antiinflammatory therapy in this cohort of patients; have been developed and tested individualized approaches to the basic treatment during the period of the disease without attacks, oriented to the correction of eosinophil-mediated inflammation of the bronchi at the absence of genes GSTT1 and GSTM1 deletion, as well as neutrophil-caused inflammation of the respiratory tracts with genotype GSTT1+M1-, GSTT1-M1+ or GSTT1-M1-.
