The thesis is dedicated to increasing the effectiveness of diagnosis and treatment of patients with paroxysmal and persistent form of atrial fibrillation. In the current study examined the clinical and functional parameters as predictors of failure of sinus rhythm, markers of inflammation and fibrosis in the dynamics of complex treatment of patients with the inclusion of the direct renin inhibitor therapy of aliskiren. We examined 100 patients with paroxysmal and persistent atrial fibrillation. In 32 (32%) of patients diagnosed with paroxysmal AF and in 68 (68%), persistent form of the disease. Obesity was observed in 34% (34 people), smoking - 21% (21 people), use of alcohol - 46% (men). 43% (43 people) reported the presence of AF in blood relatives, diabetes was diagnosed in 14% (14) of patients, the mean BMI was 29,19 ± 0,51. Clinical diagnosis of AF was installed as recommended by the ACC / AHA / ESC (2012) for the diagnosis and treatment of atrial fibrillation on the basis of specific complaints of patients, collected by medical history, taking into account risk factors (family history, presence of hypertension, smoking, etc.), the objective data and instrumental examinations: electrocardiography, and Holter ECG monitoring, biochemical blood analysis. The treatment used in therapy include aliskiren (Rasilez) for 6 months. The patients had no valvular lesion, acute coronary syndrome, thyroid dysfunction, cancer, heart failure class IV. Inclusion in the treatment scheme of direct renin inhibitor aliskiren (Rasilez), has improved the clinical course of disease, normalization of transmitral blood flow and biochemical parameters in patients with atrial fibrillation.
