Essay

The dissertation is devoted for optimization of diagnostic and therapeutic approaches in patients with residual arterial hypertension after aortic coarctation repair. Based on conducted clinical observation of the 1070 (95,9%) patients operated during 1988-2002 it has been defined the group of 309 (28,9%) patients with residual arterial hypertension, it has been defined the risk factors of the arterial hypertension appearance in the long-term period as well as the major complications of such type of the patients. The complex protocol of the diagnostic and treatment the patients after aortic coarctation repair, which will allow to asses the grade and to discover the mechanism of the residual arterial hypertension as well as to choose beforehand relevant pathogenetic therapy and to check its efficiency, has been developed first time and introduced to the clinical practice. It was developed the algorithm of the treatment and dispensary observation the patients with arterial hypertension after aortic coarctation repair. It has allowed to achieve normalization of arterial hypertension of 99,6% of the patients at the level of 128/80 mm. Hg. Art. and reliably reduce left ventricular hypertrophy and microalbuminuria of a such category of patients.
