Essay

The thesis is dedicated to improving treatment outcomes of patients with radial nerve injuries at different levels by using the optimal surgical techniques. The work is based on the analysis results of the study and treatment of 138 patients with damage to the radial nerve at different levels (about 18% of patients with injuries of peripheral nerves of the upper limb) that were on the examination and treatment in the clinic of restorative neurosergury SI "Institute of Neurosurgery names after acad. A. P. Romodanov NAMS Ukraine" (37 patients) and the Department of Neurosurgery, Rivne, RCCH (101 patient) period of 2005-2010. All patients, depending on the level of physical harm, were divided into 4 groups: 1st group - patients with radial nerve injuries in the subclavian area, at the axillary area and the upper third of the shoulder; 2nd group - patients with radial nerve injuries at the middle third of the shoulder (in the region of the spiral channel); the 3rd group - patients with radial nerve injuries at the lower third of the shoulder-elbow-joint upper third of the forearm; the 4th group - patients with radial nerve injuries in the area of supirnatornoho channel. When performing surgery in optimal terms after radial nerve injury functionally possible to achieve a positive result in nevrolizi in 87% of cases, the performance of nerve suture - 71% autoplasty - 62%. Direct neuroticism may be the method of choice in the surgical treatment of radial nerve injury. Better radial nerve recovery than other upper extremity nerve trunks connected not with the processes of radial nerve regeneration, and with features restore the functional state of muscles innervated by this nerve. Reconstructive surgery on the nerve roots are justified even under the old radial nerve injuries and to achieve positive results in 60% of cases. Long-term electrical stimulation using implanted systems is the method of choice in the treatment of old and inefficient for restoration of radial nerve.
