Essay

The dissertation is devoted to the treatment of longstanding atrial fibrillation -widely distributed disturbance of cardiac rhythm in the general population which can lead to the serious complications. Depending on the type of surgery performed, patients were divided into 2 groups. The first group of patients (n=77) performed the procedure as described C.Pappone (1998), the second (n=83) - extended procedure-stage modification of the electrophysiological substrate (SMES). The study using an activation card on-CFE-mean failed to register the CFA zones in 100% of patients. Intraoperative sinus rhythm was as follows: 43.6% in the subgroup 1a; 47.4% in subgroup 1b; 82.5% in the subgroup 2a and 86% in the subgroup 2b. Restoration of SR in the subgroup 2b, after PVI, is 30.2%, more ablation roof LA improves the efficiency of the procedure to 34.9%, MI ablation improves the efficiency of the procedure prior to 41.9%. Expanding the scope of the CRA at CS, LAS and back of LA has improved the efficiency of the method to 81.3%. Additional impact on the CFA zone added another 4.7% effectiveness of the procedure. Thus the restoration of SR in the subgroup 2b is 86% versus 43.6% in the subgroup 1a (p <0.05).
