Essay

The thesis is focused on studying the impact of serum elevated uric acid level on the formation of endothelial dysfunction in adolescents with arterial hypertension (AH) to improve methods of preventing the development of vascular complications in these patients. It has been established that in adolescents with AH, especially in the patients with obesity against hyperuricemia, the signs of maladaptive biventricular remodeling become more pronounced with formation of both diastolic and systolic myocardial dysfunctions. Moreover, glomerular filtration rate is decreased, and tubular reabsorption and microalbuminuria rates are increased. Factors, which predict the formation and further development of endothelium-related vasodilation in adolescents with AH, are hyperuricemia as well as the findings of the total amount of nitric oxide metabolites in the blood plasma and ?-2 microglobulin in the blood serum. The issues on the necessity of inclusion of the prescribed drugs, which contribute to the reduction of uricemia rates and stabilization of endothelial function, into the set of treatment and preventive measures in adolescents with AH are substantiated in the thesis.
