The thesis is dedicated to improving the treatment effectiveness of patients with skin melanoma through the introduction sentinel lymphadenectomy. The object of study were 309 patients with melanoma of extremities and trunk thicker than 1.0 mm by Breslow in the absence of clinical signs of metastases in regional lymph nodes.The melanoma thickness 1,01-2,0 mm is associated with a statistically significant decrease of metastases in SLN (p=0,0001), the thickness more than 4,0 mm - with an increase (p=0,005). At the threshold thickness of primary tumor 2,8 mm likelihood of metastasis in SLN increases 6,2 times compared with lower rates (OR = 6,2; p=0,0001).
