Essay

The thesis is dedicated to the diagnostic and treatment optimization of paroxysmal autonomic failure of children on the basis of pathological changes in the cervical spine. The paper presents the results of clinical-laboratory and instrumental studies conducted on the basis of 147 patients with paroxysmal autonomic failure, 92 of which had cervical spine pathology. The structure of cervical spine pathology is researched; the risk factors for its formation were identified. We found marked features of clinical manifestations of paroxysmal autonomic failure depending on the presence of pathological changes in cervical spine. For the first time the influence of tonic disorders in neck muscles and shoulder girdle on the course PAF was investigated. For the first time on the basis of clinical and instrumental criteria we developed a diagnostic algorithm, which allows evaluating the necessity of optimal diagnostic measures for more individual further treatment of PAF at children with cervical spine pathology. The first time it was proposed a treatment scheme of PAF at children with cervical spine pathology, using metameric reflexotherapy with the introduction at gradient reflex zones of C1-C6 segments micro-doses of drugs. It was proven the effectiveness and safety of a 2-course metameric reflexotherapy at the background of basic and general 6-month preventive treatment for achieving sustainable positive effect. We developed an algorithm of the differentiated choice of therapeutic tactics.
