Essay

Object: acute injuries of ligamentous apparatus of the lateral ankle. Aim: to improve the results of treatment of patient with acute injuries of the ligamentous apparatus of the lateral compartment of ankle by improving diagnostics and developing methods for surgical repair of ligamentous structures under arthroscopic control. Methods: clinical, radiographic, ultrasound, magnetic resonance, patent information, statistics, biomechanics, experimental, comparative analysis of clinical material. In experimental studies on cadaveric material new data about spatial relationships of bone and nerve structures of the lateral ankle were obtained. It is allowed to determine safe areas to restore lateral ligaments of the ankle under arthroscopic control. In experimental studies on cadaveric material new data about strength of the ligament reconstruction of the lateral ankle using a U-shaped suture and anchor fixing device were obtained. It is allowed to prove a new technique for restoration of acute injuries ligamentous apparatus of the lateral ankle under arthroscopic control. The proposed scheme of diagnostics allowed to determine the differential indications for surgical repair of the ligaments of lateral ankle and conservative treatment of this group of patients. New data about the topography of the injuries of the anterior talofibular and calcaneofibular ligaments which reveal during arthroscopy were obtained. That allowed the development of differential indications for the use of open and arthroscopic surgical techniques depending on the location damage. Developed differential indications for opened and arthroscopic methods of surgical treatment of acute injuries of ligamentous apparatus of the lateral ankle depending on disruption localization allowed to improve results of patients treatment. The introduction in clinical practice of minimally invasive techniques of arthroscopic repair of damages of of acute injuries of ligamentous apparatus of the lateral ankle reduces surgery trauma and improve the results of treatment of this group of patients (patent № 67905 Ukraine).
