Essay

The object of study: new cases of co-infection tuberculosis/HIV; the aim - increase of effectiveness of treatment of patients with new cases of co-infection tuberculosisi/HIV through improved antimyсobacterial chemotherapy for overcoming and prevention of adverse reactions from combined therapy in the appointment antiretroviral therapy during the intensive phase of chemotherapy; methods of the research: clinical, radiological, microbiological, immunological (determination of quantity of CD4+lymphocytes), statistical; novelty: the frequency of detection of patients with new cases of co-infection tuberculosisi/HIV was established if patients apply for medical assistance and during fluorographic examination. The first time the frequency and structure of drug resistance of Mycobacterium tuberculosis in patients with new cases of tuberculosis/HIV was established. It was proved that standard antituberculosis regimens of treatment have low effectiveness in patients with new cases of co-infection of tuberculosis/HIV. The application of antiretroviral therapy during the intensive phase of treatment of patients with co-infection tuberculosis/HIV was substantiated, that permits to improve the survival of patients, improves the results of antimyсobacterial chemotherapy, not significantly affecting on tolerability of treatment. The first time was proved that the inclusion of moxifloxacin in the intensive phase of chemotherapy instead of ethambutol and streptomycin can improve the effectiveness of treatment and safety of antiretroviral therapy during the intensive phase of chemotherapy.
