Zelenchuk O. Surgical treatment of patients with coronary heart disease with decreased quantity of viable myocardium.

The research is devoted to the surgical treatment of patients with coronary heart disease with severe heart failure. This work is the first national study, which summarizes known and outlined new ways of solving a scientific problem - stabilization, improve quality and extend the life expectancy of patients with coronary heart disease with severe left ventricular dysfunction by optimizing the diagnosis and surgical treatment. Evaluation studied in the course of coronary bypass operations results in these patients allowed to determine the indications and contraindications for coronary bypass operations in patients with coronary heart disease with low left ventricular ejection fraction. The studies allowed to identify risk factors for hospital mortality, the occurrence of postoperative complications and mortality in remote patients with coronary heart disease with severe heart failure. It is shown that coronary bypass surgery in patients with low left ventricular ejection fraction with multiple affected coronary arteries with good immediate and long-term results and is an effective surgical treatment for patients with coronary heart disease compared with isolated drug therapy. Developed and put into practice the algorithm choice of treatment of patients with coronary heart disease with decreased number of viable myocardium considering key pathogenic mechanisms in each patient.
