Keian D. Treatment of patients with odontogenic maxillary sinusitis with the restoration of the volume of the alveolar bone of the upper jaw in the area of the causal tooth.

Pathological changes in bone tissue of maxillary sinus anterior wall and the tissue surrounding the OAС in the case of chronic odontogenic maxillary sinusitis were studied in our work. Variants of the alveolar bone of the upper jaw in the area of the oroantral joint were defined, depending on the timing of its functioning. The methods of surgical treatment of chronic odontogenic maxillary sinusitis with oroantral joint using free bone graft from the anterior wall of the maxillary sinus and autoplasma of blood riched with the platelets and fibrin to close these joint were studied, taking into consideration these changes. This gives possibility for the reliable isolation the tissue of OAС from the oral cavity and the maxillary sinus and to create conditions to restore volume of the alveolar bone of the upper jaw in the OAС area. Histopathological study of bone tissue surrounding the OAС has shown that the character of pathological changes in its bone wall was changed because of the absence of inflammation during joint functioning up to 3 days and inflammation activity increasing during 3–14 days up to sclerotic changes while joint wall epithelization with resistant joint formation. Therefore, the OAС should be closed, combining maxillary sinus with sanitation and isolating the bone defect of the upper jaw of the external factors of the oral cavity and the maxillary sinus.
