Dissertation is devoted to optimization of diagnostics and prognosis of activation of immune inflammation and structural functional pancreatic disorders in patients with combined course of chronic pancreatitis and type 2 diabetes mellitus on the basis of studying clinical and phenotypic features of their current, level of apelin-36, tumor necrosis factor-?, parameters of carbohydrate and lipid metabolism in conjunction with remodeling of the pancreas. Patients with chronic pancreatitis were characterized by increased apelin-36 and tumor necrosis factor-? activity. Patients with combined course of chronic pancreatitis and type 2 diabetes mellitus were characterized by maximum increased apelin-36 and tumor necrosis factor-? activity. Pathogenetic influence of apelin-36 and tumor necrosis factor-? on the development of structural and functional disorders of the pancreas is determined in patients with chronic pancreatitis. Patients with combined course of chronic pancreatitis and type 2 diabetes mellitus are characterized the most dense relations between deterioration of carbohydrate metabolism and the development of structural and function reorganization of the pancreas. Effect of level apelin-36 and tumor necrosis factor-? on parameters of carbohydrate and lipid metabolism, as well as the remodeling of the pancreas is observed in this group of patients.
