Essay

Aiming on optimization of diagnosis and improving of the treatment effectiveness of gastroesophageal reflux disease (GERD) in combination with iron-deficiency anemia, the prospective randomized controlled cohort research was performed in 107 patients (including 62 patients with combined pathology, the rest was with isolated GERD) and 20 healthy persons. Clinical peculiarities (in combined pathology the leading were dyspeptic changes, in isolated GERD - heartburn) were distinguished. The pathogenic role of neuroendocrine system (more significant decrease of melatonin level and increase of gastrin in blood), associated with diminishing of patients health-related quality of life (evaluated by SF-36 questionnaire), in mechanism of dysbalance formation between factors of aggression and defence of esophageal mucus membrane (mostly level of SH-groups and activity of superoxide dismutase are decreasing in blood) was approved. A novel pathogenetic substantiation of expedience of Dalargin inclusion in standard treatment scheme for patients with GERD, most of all, with accompanying iron-deficiency anemia was proposed, this shortened the terms of clinical and laboratory remission achievement. Dynamic control on the spectrum of studied parameters may be used for objective additional evaluation of conducted treatment effectiveness.
