Essay

The thesis is devoted to diagnostics improving of glomerular apparatus and tubulo-interstitial kidney tissue by estimating the daily blood pressure (BP), cardiac remodelling and the relationship between renal morphological changes and levels of markers of kidney damage in patients with essential hypertension (EH) and chronic glomerulonephritis (HHN) and optimization of therapeutic correction of changes with angiotensin-converting enzyme inhibitor ramipril. The most typical circadian rhythm of BP in patients with EH and CGN is "non-dipper". The most common type of left ventricular (LV) remodelling among patients with EH and CGN is concentric hypertrophy. It is determined that morphological changes of glomerular and tubulointerstitial tissue are more expressive in patients with CGN and arterial hypertension. It is demonstrated that serum levels of cystatin, neutrophil gelatinase-associated lipocalin, interleukin-18 are the most effective diagnostic markers of kidney damage. Scientific data on ramipril action is completed with decrease of daily BP monitoring indexes, LV myocardial mass index, and reduction of markers of kidney damage in patients with EH and CGN.
