Essay

The dissertation is devoted to optimizing the diagnosis of arterial hypertension (AH), which is associated with prediabetes and type 2 diabetes mellitus (DM type 2) by studying the prevalence of polymorphic variants of I/D gene angiotensin-converting enzyme (ACE) inhibitors, A1166C gene receptor type 1 angiotensin II, T174M and M235T gene angiotensinogen, integrated assessment of anthropometric parameters, plasma levels of pro- and anti-inflammatory cytokines, parameters of carbohydrate metabolism. Detailed pathogenetic relationship between the presence of polymorphisms of genes renin-angiotensin system, markers of inflammation and development disorders carbohydrate metabolism in patients with AH. The increase levels of IL-18 in patients with AH, hypertension with concomitant prediabetes, and AH with concomitant DM type 2. The content of anti-inflammatory cytokines - IL-10 is increased in patients with AH, AH and prediabetes, at a time when levels of this cytokine decreased in patients with AH, which combined with DM type 2. The mathematical model of forecasting and differential diagnosis prediabetes, DM type 2 in patients with AH based on the evaluation identified the most prognostically significant factors - BMI, heart rate, levels of IL-10 glucose after a test load, HbA1c, the presence of A1166C gene AGTR1 polymorphism and M235T AGT gene polymorphism.
