The thesis paper is devoted to specific features of the clinical course and improvement of treatment of the oral lichen planus in patients with type II diabetes with due regard for indicators of the local and systemic immunity. They conducted survey of 133 patients with lichen planus aged 25-65, of whom 58 (43.6 %) had the combined pathology - type 2 diabetes mellitus (I group - basic) and 75 patients (56.4 %) - had the isolated lesions of oral lichen planus (II group - comparison). The control group was represented by 30 apparently healthy individuals. The survey was conducted before and after treatment. Survey results of 325 in-patients indicate that the oral mucosa pathology was detected in 142 (43.7 %) patients. It was found that lichen planus associated with type 2 diabetes mellitus is the most common disorder of the oral mucosa, lesions totals 17.8 % of cases. Candidiasis was recorded in 14.2 % of patients, neurogenic diseases of the tongue - in 8 % of patients, leukokeratosis in 3.7 % of cases. It is established that specific features of the clinical course of the oral lichen planus in patients with type II diabetes are determined by diabetes compensation, different options of its progress and age of patients. The author offers the new algorithm of the comprehensive treatment of the oral lichen planus in patients with type II diabetes using the immunomodulatory systemic agent Galavit and the anti-inflammatory spray Anginovag having the local effect, which allowed significant shortening the treatment duration, achieving the long-term clinical remission (1.5 years), normalizing parameters of local and systemic immunity, as well as improving the oral hygiene.
