Essay

Object: process interbody joit after performed anterior bisegmental interbody fusion in patients with injuries and degenerative diseases of the cervical spine, technologies of anterior surgical stabilization of the cervical spine. Purpose: increase the effectiveness of surgical treatment of patients with cervical spine injuries and diseases through the development and validation of advanced methods of anterior interbody fusion using hybrid dynamic translational-rotational cervical device. Methods: clinical, radiographic and computed tomography, biomechanical (mathematical modeling using finite element method), statistical. The first known results of the use of cervical dynamic devices were analyzed, its biomechanical characteristics were studied, specific complications for each of the kind were identified. For the first time using an advanced technique of radiological research, new scientific data on the dynamics of the location of cervical devices, screws and fixed cervical vertebrae after performed anterior interbody fusion were obtained. The essence of the dynamic process and its regularities have been refined using the dynamic translational-rotational cervical devices. For the first time a comparative study of stress-strain state of the system "cervical vertebral motor segments - implants" in the simulation bisegmental anterior cervical interbody fusion devices (dynamic and rigid) was carried out with the use of mathematical modeling. Its effect on this state was determined. For the first time the effectiveness of clinical application of advanced methods of anterior interbody fusion using hybrid dynamic translational-rotational cervical plates in patients with injuries and diseases of the cervical spine of patients has been studied. System X-ray evaluation of vertebral motor segments, fixed by means of cervical device, was developed and implemented. This system reflects the quality and quantity of the vertebrae position in fixed vertebral motor segments, devices, screws and interbody support and dynamic of interbody fusion in patients with injuries and diseases of the cervical spine after performed anterior interbody fusion. Methods of anterior interbody fusion using dynamic translational-rotational device has been developed and introduced into practice, thus allowed to improve the indicators of interbody fusion and results of treatment. Results of the study were introduced into clinical practice in SI "Sytenko Institute of Spine and Joints Pathology National academy of medical sciences of Ukraine", traumatology and traumatology-orthopedic departments of Community Health Institutions "Kharkiv Regional Clinical Trauma Hospital", Community Health Institutions "Odessa Regional Clinical Hospital", Community Health Institutions "Kherson Regional Clinical Hospital".
