Essay

The thesis concerns problems of modern neurology and neurosurgery — improving treatment results of patients with brain abscesses with modern stereotaxic and neuronavigation technology. The paper presents the results of treatment of 242 patients with brain abscesses from various regions of Ukraine from 1994 to 2014. Improved methods of diagnosis and surgical give positive tendency of quality improvement of treatment of patients with brain abscesses from 1995 to 2014 in different regions of Ukraine. Analyzing the results on years of research, traced a clear tendency to reduce the percent of deaths from 20% in 1995–1999 respectively to 9,9% in 2011–2014 respectively, and accordingly increase positive results. In the past decade has increased the percentage is good that in 1995–1999 amounted to only 30% in 2000–2005 years – 27,5%, in 2006–2010 years — 28,4%, in 2011–2014 years are 40,8% of cases. Developed and implemented in practice empirical treatment regimen of patients with abscesses of the brain depending on the mechanism of infection. Our studies show that the best results were obtained using stereotaxic modern technologies. In the group of patients who were operated on using stereotaxic technology fatalities were observed in patients operated by conventional surgical techniques (without neuronavigation) mortality was 11,1%. Accordingly good results when using neuronavigation was obtained in 50%, while traditional interventions 30,8% (P <0,05). These studies suggest that stereotactic methods are the method of choice for today, especially when located deep abscesses, and those located in important functional areas of the brain. In multi abscesses, abscesses anaerobic etiology, with the presence of foreign bodies and clinics with limited technical capabilities of the total removal of the abscess may be the best method of treatment.
