The thesis is devoted to increasing the efficiency of predicting the course of bronchial asthma in children on the basis of studying the functional state of vascular endothelium and vascular factors of adhesion and inflammation. Pathogenic significance of expression of cell-cell adhesion sVCAM-1 molecule in formation of inflammatory process as well as in activation of endothelium dysfunction in children suffering from bronchial asthma was outlined. Presence of relationship of sVCAM-1 level with instrumentaland biochemical indices off endothelial function, external respiration function and inflammation indices was estimated. Dependence of intensity of bronchoobstructive syndrome on functional state of vascular endothelium and expression of cell-cell adhesion sVCAM-1 molecule was proved. The multiple logistic regression analysis made it possible to carry out mathematical modelling of probability of development of severe course of the disease. The factors, obligatory for prognosis of formation of severe course of bronchial asthma (positive family allergic history, blood serum sVCAM-1 and NO2+NO3levels, intima-media complex thickness of the common carotid artery), were established.
