Kravtsiv V. Heart failure in patients with toxic goiter. Optimization of diagnosis and prediction of remote treatment outcomes

The goal and objectives were resolved by assessing tolerance to physical activity, heart rhythm disorders, structural and functional changes in the heart and in blood of natriuretic propeptide in patients with toxic goiter and hyperthyroidism on the background of thyrotoxicosis and euthyroidism with medication and treatment. We studied the diagnostic value of the applied testing, optimizing predictors of heart failure diagnosis, prediction of long-term results of treatment of hyperthyroidism. Optimizing diagnostic predictor of severity of heart failure is the length of distance in 6 minute walk test, oxygen consumption value, percentage of increase in heart rate and breathing, debt scale scores, duration of hyperthyroidism, the level of blood N-terminal brain natriuretic propeptide. Long-term results of treatment depend on the severity and duration of hyperthyroidism and severity of its complications. The method of estimating the long-term results of treatment was developed. In moderately severe thyrotoxicosis good and satisfactory results were obtained in 98.0 % of patients and severe – in 44.1 %. The algorithms of forecasting long-term results of treatment of patients with toxic goiter were worked out.
