Essay

The results of clinical and instrumental examination and treatment of 117 patients with occlusive-stenosis pathology extracranial carotid arteries that were treated in State Institution "Institute of Neurosurgery named after acad. AP Romodanov NAMS Ukraine " during 2006-2016 years. The age of patients ranged from 45 to 85 years, women accounted for 13,6% (n=117), men - 86,4% (n=74). All patients were operated on occlusive-stenosis pathology extracranial carotid arteries by the method of surgical treatment were divided into two groups: patients who performed surgery (CEA) and patients who performed endovascular treatment (CAS). Surgery held 36,7% (n=117) patients, 97,6% (n=43) performed classical carotid endarterectomy in - 2,4% (n=43) performed classical carotid endarterectomy using the patch. Endovascular treatment was made 63,3% (n=117) patients, 78,3% (n=74) conducted a unilateral carotid angioplasty and stenting of stenosis in 21,7% (n=74) conducted a simultaneous bilateral carotid angioplasty and stenting. The level of quality of life and social adaptation of patients before discharge from hospital and at the test examinations were evaluated by the Glasgow Outcome Scale (GOS), modified Rankin Scale (mRS) and created our questionnaire. Based on analysis of patients with occlusive-stenotic pathology of extracranial carotid arteries we developed criteria for application of differentiated surgical or endovascular treatment technologies in patients with cardiovascular risk.
