Essay

In study conducts theoretical and practical research of the psychiatric problem of differential diagnostic criteria, instruments and formal algorithm between endogenous catatonia and catatonomorph violations of different etiology. Diagnostic methodology formed basing on recognition and analysis of clinical semiotics, structural and dynamic features and pathomorphosis trends. 4 groups of pathomorphosis factors of catatonic syndrome were separated and systemized on 2 vectors: functional (pharmacogenic and toxic) and morphological (alterative and involutive). Formed typology of catatonia pathomorphosis which includes 4 basic types of catatonic syndrome deviations, their population frequency was found as well. Differential diagnostic tablets was formed, creation "Scale of endogenous catatonia recognition" with 3-step realization procedure. Developed scale allows diagnostic conclusion between (endogenous) catatonia and catatonomorph violations of pharmacogenic, dissociative and organic genesis. Basing on developed scale the clinical algorithm of endogenous catatonia differential diagnostics was formed. Its value determined by 94.43% of true positive and 90% true-negative, 5.56% of false-positive and 10% of false-negative diagnostic decisions respectively.
