Essay

The dissertation is devoted to the problem of increasing the effectiveness of surgical treatment of rhegmatogenous retinal detachment with extrascleral methods. Supplemented scientific data on the state of hemodynamic disorders in rhymatogenous retinal detachments. New data on the state of hemodynamics in the eyes with rhegmathogenous retinal detachment after surgical blocking of retinal gaps were obtained, namely: hemodynamic disorders were deepened and the dependence of restoration of visual functions on the state of blood circulation in the eye was first proved. For the first time, the scientific feasibility of the complex application of extraversal methods for treating rhymatogenic retinal detachments with correction of hemodynamic disturbances in the eye, which can be achieved by the one-time implementation of extra-scleral interventions and indirect-revascularizing operations, is substantiated. A new complex method of surgical treatment of regmatctomy retinal detachment with extraversal methods has been developed, which includes components for breaking the gap and correction of microcirculatory disturbances by carrying out operations of indirect revascularizing action (ligation of the surface temporal artery or ligation of the facial artery), which allowed not only to achieve the anatomical fit of the retina, but also to increase linear and volumetric blood flow rates by 8.5 - 12.5% (p<0,05). Introduction to the clinical practice of the proposed method contributes to the improvement of functional results of surgical treatment of such conditions - provides increased visual acuity by 20% (p<0,001).
