Essay

In the thesis, a theoretical summarization of the results is presented and a solution of the current scientific problem is achieved on the basis of a survey of 96 patients with osteoarthritis (OA) with concomitant obesity, 18 patients with isolated OA and 96 controls selected on the basis of the "copy-pair" principle. The pathogenetic role of VDR and LCT gene mutations and the effect of increased serum adipocytokine apelin-13, which in the complex can lead to worsening of the severity of OA in patients with obesity, and contribute to the formation of osteopenia in these patients, are determined and proven. Based on the definition of clinical and anamnestic data, indices of algo-functional activity (WOMAC index), x-ray stage of osteoarthrosis, BMI, adipokine-13 level, and polymorphism of VDR and LCT genes, stratification groups of young patients with OA and obesity with different the degree of risk of development of bone mineral density disorders (BMD) are formed, that made it possible to predict the course of the disease, as well as control future treatment. Prognostic coefficients of individual clinical and genetic risk factors have been created and tested, and an algorithm for assessing the individual risk of structural and functional bone disorders in patients with osteoarthritis and obesity has been developed.
