Essay

The thesis is devoted to the study of clinical features and mechanisms of the development of chronic pancreatitis (CP) combined with obesity and type 2 diabetes mellitus depending on the insertional-deletion (Ins / Del) polymorphism of the apolipoprotein-B gene (Apo-B) and increasing of the treatment efficiency of this disease in the context of lipid metabolism disorders. The frequency of deletion of the functional zone of the apolipoprotein - B gene and clinical symptoms in patients with chronic pancreatitis combined with obesity and type 2 diabetes mellitus has been determined. The data on increasing the content of glucose, IRI, HOMA-IR, HbA1C, atherogenic lipid fractions and the imbalance of adipocytokines in blood plasma were clarified, and for the first time the risk of CP, obesity and type 2 diabetes mellitus with high content of atherogenic lipid fractions was defined. For the first time there was proved the reasonability of inclusion of combination of simvastatin and ezetimibe in the basic therapy of patients with CP combined with obesity and type 2 diabetes mellitus that contributed to a decrease in LDL cholesterol level, TG, the Atherogenic Index and an increase in HDL cholesterol level, regardless of the alleline state of the apolipoprotein-B gene
