The dissertation devoted to the optimization of the management of coronary artery disease in patients with type 2 diabetes mellitus based on the evaluation of fractalkin and asymmetric dimethylarginine role in vascular lesions, as well as improving the treatment of these patients by studying the feasibility and effectiveness alpha lipoic acid in addition to the standard therapy. The connection between fractalkin and asymmetric dimethylarginine and the parameters of vascular lesions (intima-media thickness of common carotid artery, carotid-femoral pulse wave velocity, presence and severity of atherosclerotic lesions of the coronary arteries) were detailed. A method for measuring carotid-femoral pulse wave velocity using a four-channel reographer has been developed and clinically tested. High prognostic significance of determination of fractalkin, asymmetric dimethylarginine, intima-media thickness of common carotid artery, carotid-femoral pulse wave velocity in diagnosing the prevalence of coronary atherosclerosis has been established. It has been shown that alpha lipoic acid administration in addition to standard regimen helps to reduce endothelial dysfunction (reduction of fractalkin levels, asymmetric dimethylarginine levels), lipid metabolism improvement and improved vascular elastic properties.
