The study is devoted to optimization of early diagnostics and prognosis of glucometabolic disorders and predictors of type 2 diabetes mellitus in patients with hypertension and obesity basing on determination of nesfatin-1 plasma level, parameters of carbohydrate and lipid metabolisms. A high level of nesfatin-1 in patients with hypertension has been established. It was found that reduction of nesfatinemia level in hypertensive patients is associated with the appearance of abdominal obesity. The participation of nesfatin-1 in changes of anthropometric parameters and glycemic profile at its maximum concentrations in patients with a comorbidity of hypertension and obesity was emphasized. The dependence of carbohydrate metabolism parameters on the level of nesfatinemia in patients with comorbid hypertension and obesity, which is maximal in the absence of the carbohydrate profile disturbances or in the presence of prediabetes, is demonstrated. Reduced levels of postprandial glycemia with higher rates of nesfatin-1 in plasma of patients with hypertension and prediabetes were determined. A correlation was found between the level of nesfatin-1 and carbohydrate metabolism indexes in patients with hypertension with concomitant type 2 diabetes mellitus and normal anthropometric parameters. In patients with a comorbidity of hypertension, obesity and type 2 diabetes mellitus, there is a lack of correlation between the level of nesfatinemia and metabolic parameters.
