Scientific research is devoted to optimization of diagnostics of endothelial dysfunction in patients with chronic obstructive pulmonary diseases with concomitant arterial hypertension basing on evaluation of nitrogen oxide system, catecholamines levels, vasculoendothelial growth factor, and levels of biogenic elements, vasoactive hormones, peptides and neuromediators, and possibility of their use in prognostication of severity of state of patients with comorbid pathology. Presence of changes in nitrogen oxide system in patients with chronic obstructive pulmonary disease complicated by addition of AH has been determined. Changes in functioning of proteolytic system of blood is found, most informative were ACE and carboxypeptidase N. Found high activity VEGF and significant reduction of VIP, which proves pathogenetic role of endothelial dysfunction in formation of comorbid pathology. Demonstrated changes in the catecholamines in COPD. Maximal changes were present in AH. Found formation of diastolic dysfunction of both ventricles and development of hypertrophic types of remodeling in patients with COPD and AH. Basing on mathematical statistical analysis of data were developed classification functions, which give possibility in prognostication of severity in patients with comorbid pathology.
