The thesis is devoted to the study of increasing the effectiveness of diagnostics and treatment of patients with acute myocardial infarction with concomitant obesity on the basis of the study of the pathogenetic role of osteopontin and thrombospondin-2 in the left ventricle remodeling. The study revealed that the features of the acute coronary syndrome development in obese patients are the hyperactivity of the adhesion modulator thrombospondin-2 and the high modulatory activity of such direction from osteopontin together with its proinflammatory effects. The efficiency of the complex application of zofinopril and spironolactone in the treatment of patients after acute myocardial infarction is proved despite the progression of the abdominal type of obesity, which positively affects the activity of the regulators of the configuration of the extracellular matrix and the parameters of the structural and functional state of the left ventricular myocardium. A model for predicting the development of Q-positive myocardial infarction in patients with acute coronary syndrome with elevation of the ST segment in the presence of concomitant obesity, based on the analysis of the prognostic value of thrombopon-din-2 and routine laboratory data of patients was developed.
