The work is devoted to the development of approaches to the optimization of therapy and rehabilitation of patients with paranoid schizophrenia, which is based on the definition of their clinical and functional gender specificity and principles of clinical and functional diagnosis reasoning, taking into account the peculiarities of clinical and psychopathological manifestations of paranoid schizophrenia in men and women, and a comprehensive assessment of their psychosocial and family functioning differences. On the basis of the results of a comprehensive examination of 420 patients with paranoid schizophrenia, which were divided into two groups, namely 210 men and 210 women, the clinical and anamnestic features of patients with paranoid schizophrenia were considered in the light of the gender factor; a comparative analysis of the characteristics of the positive, negative and general psychopathological symptoms of paranoid schizophrenia in men and women was carried out; taking into account the gender factor of patients with paranoid schizophrenia the clinical and social features of patients, as well as the specific quality of their life and family functioning, were established in the work. The principles for determining the clinical and functional characteristics of patients with paranoid schizophrenia were developed as well as ways of optimizing their therapy were suggested. Taking into account patients’ clinical and functional gender peculiarities, differentiated approaches to their psychosocial rehabilitation were developed. The assessment of the effectiveness of the proposed measures showed their effectiveness compared with standard measures, namely, in the reduction of psychopathological symptoms and the improvement of the patients’ quality of life.
