Essay

Anemia in HIV-infected people is widespread, an independent cause of disease progression and a significant increase in the mortality risk. The main role in the development of anemia among HIV-infected individuals belongs to the deceleration of erythropoiesis, as well as disturbance of iron metabolism in a human body. In most cases, anemia in HIV-infected individuals is a multifactorial process that includes iron deficiency and disturbance of its distribution, the impact of opportunistic infections and drugs on the HIV infection treatment. Pathogenesis of anemia among HIV-infected individuals is rather complex and has not been completely studied. In order to conduct a rational diagnostic search and timely adequate treatment of anemia, a clear understanding of the possible options of the development of anemia in HIV infection is necessary. The use of the drug containing FeII sulfate sekodrat in the HIV-infected people contributed to the regression of the clinical symptoms of anemia and the normalization of erythropoiesis in them in the absence of a positive therapeutic effect in the patients with HIV/TB coinfection. The obtained results enabled to substantiate the use of a drug containing FeII sulfate sekodrat for the correction of anemia in HIV-infected people. At the first time, a complex study of erythropoiesis indicators in HIV-infected individuals and patients with HIV/TB co-infection has been done. Significant deviations in the content of erythropoiesis indicators in patients with HIV/TB co-infection in comparison of HIV-infected individuals and control established, this data confirmed more severe erythropoiesis disorders in patients with a combined infection. Comprehensive analysis of clinical and laboratory parameters in patients with anemia on the background of HIV infection allowed to development diagnosis criteria of anemia in HIV-infected and patients with co-infection HIV/TB. The algorithm of clinical and laboratory diagnostics of anemia and approaches to correction of determined violations of erythropoiesis in HIV-positive persons was created and implemented in practice. The efficacy of the bivalent iron drug in the treatment of anemia in HIV-infected patients has been shown. The drug contributes to significant positive changes of the main clinical symptoms and erythropoiesis indicators.
