Object of research: the first diagnosed pulmonary tuberculosis (FDPTB); research methods: clinical, radiological, microbiological, instrumental, statistical; novelty of work: new scientific data have been obtained that microalbuminuria (MAU) of varying degrees of severity and elevation of liver enzymes and bilirubin are signs of hepatorenal dysfunction in patients with FDPTB; The relationship between the level of microalbuminuria and prevalence of tuberculosis process and the presence of intrahepatic cholestasis, cytolysis of hepatocytes in patients FDPTB; an increase in the severity of MAU and the emergence of signs of cytolysis of hepatocytes and intrahepatic cholestasis in patients with FDPTB, who received standard antimycobacterial therapy (AMBT) within 2 months without the use of pathogenetic therapy; scientifically substantiated the appointment of cytoprotective therapy on the basis of L-arginine and silibinin in addition to standard AMBT, which contributes to the reduction or elimination of signs of hepatorenal dysfunction; algorithm for management of patients first diagnosed with pulmonary tuberculosis complicated hepatorenal dysfunction and purpose of immunosuppressive therapy based on L-arginine and silibinina addition to standard AMBT.
