Essay

The thesis is devoted to the improvement of diagnosing, treatment and prognostication of the course of coronary heart disease (CHD) after community-acquired pneumonia (CAP). The peculiarities of structural-functional parameters of the heart, heart rate variability, and changes in biomarkers of proinflammatory activation and endothelial dysfunction in patients with CHD after CAP are established. Independent predictors of cardiovascular events are determined and the method for predicting the incidence of CHD in patients after CAP is developed by using a discriminant equation. It is proved that inclusion of L-arginine into the basic therapy of CHD improves the state of cardiohemodynamics, autonomic regulation of the heart, reduces the severity of myocardial ischemia and is associated with a more favorable course of the disease during 1 year of observation.
