Bairamov E. Total cavopulmonary connection in the surgical treatment of congenital heart disease with single ventricle.

The dissertation is dedicated to the problem of surgical treatment of patients with a single ventricle morphology. The thesis presents the analysis of 130 patients with single ventricle defects, in whom extracardiac TCPC were performed in GI «The scientific-practical medical center of pediatric cardiology and cardiac surgery of Ministry of Health of Ukraine» from 2007 to 2014. The present work is the first study in Ukraine that summarizes known and implementes new solutions to scientific objectives - to improve the results of surgical treatment of patients with SV by enhancing diagnostics and methods of surgical correction, justifiing tactics and postoperative management strategies. The comprehensive analysis of the results allowed to determine the risk factors for mortality, early and late complications after the TCPC. For the first time a conceptually new approach for the prevention of severe postoperative complications was introduced and new surgical modifications were implemented: a modified technique for creating fenestration, EC-TCPC without the use of CPB and creation of the distal anastomosis with fully offset planes of caval anastomoses.
