Object of study: Chemo-resistant tuberculosis (СRTB) of the lungs; is aim – to increase the efficiency of treatment of patients with newly diagnosed CRTB of the lungs by early diagnosis of chemo-resistant molecular genetic method, early correction of antimycobacterial therapy and correction of disorders of the immune and antioxidant systems; methods of investigation: general-clinical, X-ray, molecular-genetic, microscopic and culture methods, immunological, laboratory, statistical; the novelty: scientific data on the early diagnosis of chemo-resistant mycobacterium tuberculosis (MBT) by the molecular genetic method and, accordingly, the early correction of antimycobacterial therapy, which increases the efficiency of treatment of patients with CRTB, is refined; It was established that early diagnosis of MBT chemo resistance and early correction of antimycobacterial therapy in patients leads to restoration of indicators of immune and antioxidant systems of the organism; it has been proved that treatment of patients with CRTB in diagnosis of MBT resistance to antituberculous drugs by culture methods and correction of treatment in 2 months does not lead to restoration of indicators of immune and antioxidant systems of the organism; the necessity of early application of therapy aimed at restoring the state of the immune system of the organism and its antioxidant protection in the treatment of patients with CRTB of the lungs based on the diagnosis of chemo resistance culture methods is proved; the scheme of the use of corrective therapy with the addition of tocopherol acetate and tincture of echinacea in patients with primary resistance to antituberculous drugs was developed, which increases the effectiveness of treatment by 18,1 % due to the early termination of bacterial exclusion and the closure of destruction cavities by 13,3 %.
